The traditional paradigm for orthognathic surgery includes presurgical orthodontic treatment, surgery, and postsurgical orthodontic treatment. The surgery-first approach bypasses the presurgical orthodontic phase and has been demonstrated to reduce overall length of treatment. One of the challenges associated with the surgery-first approach is that it can be difficult for the surgeon to anticipate surgical movements and set the final occlusion without the patient having been orthodontically decompensated first. We will review how the use of virtual orthodontic decompensation can help the OMS plan orthognathic surgery as well as pearls for success using the surgery-first approach.
